Rozumiem, że ani szkoła, ani nauczyciele nie ponoszą żadnej odpowiedzialności za biezpieczeństwo dzieci podczas wszelkich imprez czy zajęć szkolnych (zabawy, wycieczki, pikniki, etc.)
(I hereby declare that my child will attend classes at the Lakewood Polish School during the 2007/2008 academic school year. As the parent/legal guardian of the minor (minors) for whom this application submitted, I agree to release, indemnify, and hold harmless the Polish School, its directors, appointees, and representatives from any claim arising from said minors participation in the school sponsored activities and from any claim arising from participation of my family in the school sponsored activities.
